• To enroll a student at Taylorsville Elementary School, please bring the following items to our office between the hours of 9:00 a.m. and 12:00 p.m. for your child to begin school the next day.
    •  Birth Certificate
    •  Immunization Record
    •  Proof of Residency - This must be a gas, water, or electric bill with a parent or guardian's name. If you don't have one of these bills in your new address, we can use a Purchase, Lease, or Rental Agreement as long as the property address is on the document.
    During the enrollment process, there are several other forms we will ask you to complete. These include:
    • Application for Free or Reduced Meals and Textbooks - Did you know that if you qualify for this assistance, you also help the school? We have attached information on the requirements. We encourage you to sign BOTH sections of this form!  Information Sheet   Application Form
    • Criminal Background Check - All Classroom/School Volunteers and Field Trip Chaperones MUST have this form on file in the Office before you can volunteer or go on a field trip. These are good for 1 year. Criminal History Form - English       Criminal History Form - Spanish
